ABSTRACT

Evaluar la influencia del operador sobre el transporte apical y centrado de la instrumentación endodóntica utilizando WaveOne Gold en canales radiculares simulados de resina acrílica. Se asignaron 32 canales simulados de resina acrílica a dos grupos (n = 16). El grupo 1 conformado por estudiantes inexpertos y el grupo 2 por especialistas en el área de endodoncia. Estos canales se instrumentaron hasta un tamaño apical de 25/07 con lima Primary WaveOne Gold. Para evaluar el transporte apical y el centrado de la instrumentación se utilizaron imágenes fotográficas que fueron analizadas mediante los software de edición de imagen Photoshop e Image J. Además fue registrada la incidencia de fractura de instrumentos. Los datos fueron registrados mediante un formulario Google Forms y analizados mediante el software estadístico R. Se evaluó la normalidad de los dato s mediante el test Shapiro-Wilk y para establecer diferencias entre los grupos se realizó la prueba t. El nivel de significancia estadística fue establecido en p < 0,05. La mayor diferencia al evaluar el centrado se evidenció a nivel de los 6 mm y en cuanto al transporte apical la mayor discrepancia fue a nivel de 1 mm. Sin embargo, no se observaron diferencias significativas entre ambos grupos e n ninguno de los puntos evaluados. Se reportó una baja incidencia de fractura de instrumentos. La experiencia del operador no influye en la conformación del canal radicular simulado al evaluar centrado y transporte apical al utilizar limas WaveOne Gold.


To evaluate root canal transportation and centering ability as influenced by operator experience using WaveOne Gold files in simulated acrylic resin root canals. Thirty-two simulated acrylic resin canals were distributed to two groups (n = 16). Group 1 is made up of inexperienced students, and group 2 by specialists in the field of endodontics. These canals were shaped to an apical size of 07/25 with the Primary WaveOne Gold file. Photographic images were used to evaluate apical transport and centering of the instrumentation, which was analyzed using Photoshop and Image J image editing software. File separation was also recorded. The data were recorded using Google Forms and analyzed using the R statistical program. The normality of the data was evaluated using the Shapiro-Wilk test, and the t-test was performed to establish differences between the groups. The level of statistical significance was established as p <0.05. The normality of the data was evaluated using the Shapiro-Wilk test. Then statistical significance was analyzed using the T-test. The most remarkable difference when evaluating centering was evident at the 6 mm level. In terms of apical transport, the major discrepancy was at the 1 mm level. However, no significant differences were observed between both groups in any of the points evaluated. A low incidence of instrument fracture was reported. According to our results, the operator's experience does not influence shaping in simulated root canals in terms of centering ability and apical transportation when using WaveOne Gold files.

ABSTRACT

Objetivo: Evaluar radiográficamente tratamientos en- dodónticos del sector posterior de la cavidad bucal y com- parar la frecuencia de aceptabilidad en cada pieza dentaria involucrada. Materiales y métodos: Se evaluaron 5000 radiogra- fías de archivo de tratamientos endodónticos realizados apro- ximadamente entre 2005 y 2019 en premolares y molares mandibulares y maxilares en Argentina. Se consideraron tratamientos correctos e incorrectos de acuerdo con: 1) con- formación de la preparación quirúrgica; 2) límite apical de la obturación; 3) homogeneidad de la obturación. Se obtuvo la fre- cuencia absoluta y relativa de correctos e incorrectos. El aná- lisis entre frecuencias y pieza dentaria se realizó con la prueba de chi cuadrado y el cálculo del coeficiente V de Cramer. Para la comparación entre grupos de piezas dentarias se utilizó la partición del valor de chi cuadrado obtenido en los corres- pondientes grados de libertad. Nivel de significación P <0.05. Resultados: La partición del valor de chi cuadrado no mostró diferencias significativas entre primeros y segundos premolares mandibulares. Las otras comparaciones exhibie- ron diferencias significativas. Conclusiones: Un alto porcentaje de los tratamientos endodónticos de la población estudiada tiene por lo menos una condición que permite categorizarlos como incorrectos según el criterio establecido en este trabajo. Este porcentaje es más preponderante en anatomías complejas (AU)


Aim: To evaluate radiographically endodontic treat- ments performed in the posterior area of the oral cavity and compare the frequency of acceptability in each tooth involved. Materials and methods: 5,000 archival radiographs of endodontic treatments performed between 2005 and 2019 on mandibular and maxillary premolars and molars in Argentina were evaluated. The percentages of correct and incorrect treatments were considered according to 1) shaping of the preparation; 2) apical limit of the obturation; 3) homogeneity of the obturation. The absolute and relative frequencies of correct and incorrect treatments were calcu- lated. The association between these frequencies and tooth type was analyzed using the chi-square test and Cramer's V coefficient. For the comparison between groups of teeth, the partition of the chi-square value obtained in the corre- sponding degrees of freedom was used. Level of significance was P <0.05. Results: The partition of the chi-square value did not show a significant difference between the first and second lower premolars. The differences were significant in the other comparisons. Conclusions: A high percentage of the endodontic treat- ments in the study population have at least one condition war- ranting their classification as incorrect according to the crite- ria established in this study. This percentage is more prevalent in complex anatomies (AU)

ABSTRACT

Objective:To discuss the principle and effect of augmentation rhinoplasty with auricular cartilage and expanded polytetrafluoroethylene.Methods:From January 2018 to January 2020, 161 patients (10 males and 151 females; aged from 19 to 48 years, with an average of 26 years) underwent " auricular cartilage plus expanded polytetrafluoroethylene" augmentation rhinoplasty in Nanfang Hospital of Southern Medical University. The expended polytetrafluoroethylene was carved into a willow leaf shape (I Shape) to fill the nasal dorsum, and the cartilage taking from cymba concha was constructed into an arched bridge shape for the nasal tip shaping. Pre-operative and 1-year post-operative measurements nasal length, nasal height, nasal depth, nasal columella height, nasal tip width, nasofrontal angle, nasolabial angle, survey of satisfaction and complication rate 1-2 years after operation were taken. The statistical analysis of nasal morphological indicators and nasal aesthetic indicators were employed.Results:The nose shape of 161 patients was improved to varying degree. All morphological indicators were improved, and difference was statistically significant ( P<0.05). The nasofrontal angle reached the standard in 90 cases, accounting for 55.9%; The nasolabial angle reached the standard in 143 cases, accounting for 88.8%. 2 cases had prosthesis (ePTFE) deviation and were corrected by surgical repair; 1 case had prosthesis (ePTFE) rejection and was corrected by prosthesis (ePTFE) removal surgery. Conclusions:Corresponding to the anatomical characteristics of the external nose, the prosthesis material is designed and made to correspond to the dorsum shape of the nasal stent. The shape of the alar cartilage, the prefabricated arched bridge shape of the cymba concha cartilage are used to reconstruct the nasal tip, which can effectively elevate the nasal dorsum, improve the protruding degree and rotation degree of the nasal tip, and have good long-term support. The flexibility and activity of the nasal tip are similar to the biological nose.

ABSTRACT

Introdução: a superexpressão da COX-2 relaciona-se com o aumento da produção de fatores de crescimento vascular e como consequência, com o desenvolvimento tumoral. O Firocoxib é um anti-inflamatório não esteroidal utilizado para inflamação associada à osteoartrite em cães. É o inibidor mais seletivo da COX-2, reduzindo eficientemente a ação desta enzima. Estudos indicam os benefícios do Firocoxib na terapia antineoplásica. Objetivo: este trabalho objetivou avaliar o efeito modulador do Firocoxib sobre a ação da doxorrubicina (DXR) por meio do teste de tumores epiteliais (ETT) em Drosophila melanogaster. Metodologia: foram preparadas três concentrações de Firocoxib: 2,5; 5 e 10 mg/mL, utilizadas isoladamente e em associação à doxorrubicina. O tratamento ocorreu com larvas de D. melanogaster descendentes do cruzamento de fêmeas wts/TM3 com machos mwh/mwh. Resultados: os resultados sugerem que o Firocoxib possui atividade moduladora sobre a ação carcinogênica da DXR, pois houve redução significativa nas frequências tumorais dos indivíduos tratados com diferentes concentrações de Firocoxib em cotratamento com a doxorrubicina quando comparadas à frequência tumoral do controle positivo. Conclusão: conclui-se que, nas presentes condições experimentais, o Firocoxib reduziu a frequência de tumores induzidos pela doxorrubicina em D. melanogaster.


Introduction: COX-2 overexpression is related to increased production of vascular growth factors and, as a consequence, to tumor development. Firocoxib is a non-steroidal anti-inflammatory drug used for inflammation associated with osteoarthritis in dogs. It is the most selective inhibitor of COX-2, efficiently reducing the action of this enzyme. Studies indicate the benefits of Firocoxib in anticancer therapy. Objective: this study aimed to evaluate the modulatory effect of Firocoxib on the action of doxorubicin (DXR) by means of the epithelial tumor test (ETT) in Drosophila melanogaster. Methodology: three concentrations of Firocoxib were prepared: 2.5; 5 and 10 mg/mL, used alone and in association with doxorubicin. The treatment occurred with D. melanogaster larvae descended from the crossing of wts/TM3 females with mwh/mwh males. Results: the results suggest that Firocoxib has modulating activity on the carcinogenic action of DXR, as there was a significant reduction in tumor frequencies in individuals treated with different concentrations of Firocoxib in co-treatment with doxorubicin when compared to the tumor frequency of the positive control. Conclusion: it is concluded that, under the present experimental conditions, Firocoxib reduced the frequency of tumors induced by doxorubicin in D. melanogaster.

Peripheral artery aneurysms,with low incidence and complex anatomic structure,often involve important branches.This paper introduces a new surgical technique-sleeve shaping on the basis of two cases.The basic data,including characteristics,imaging,operation and follow-up data of the cases,were collected.The data were then combined with the previous literature for explaining in detail that this technique can be used as a supplementary method of reconstruction following resection or endovascular repair.

ABSTRACT

Objective:To evaluate the clinical application effect of two nickel-titanium engine driven systems on moderate curved root canals. Methods:Ninety mesial root canals with moderate curvature of extracted human first mandibular permanent molar were randomly divided into three groups: M3 PRO Group, MTwo group and K group. The glide path was firstly prepared by using #10/04 file in M3 PRO group and MTwo group and #10 file in K group respectively.Then the preparation of root canals up to #25 was performed according to the instruction manual in each different group using standardized technique. All data including time required in glide path preparation, shaping preparation, total root canals preparation and changes of working length in root canal were recorded and analyzed statistically by using Analysis of Variance (ANOVA) and LSD-t tests (P<0.05). Results:Root canal glide path preparation time: MTwo group showed significantly less preparation time while M3 PRO group exhibited significantly the longest preparation time. There was significant difference between MTwo group and M3 PRO group (P=0.001). Root canal shaping preparation time: M3 PRO group showed significantly less preparation time while K group exhibited significantly the longest preparation time. Both MTwo group and M3 PRO group showed significantly less preparation time than K group (P=0.004, P=0.001), but there was no significant difference between MTwo group and M3 PRO group (P=0.573). Total root canal preparation time: MTwo group showed the shortest time while K group exhibited the longest time. There were significant differences between MTwo group, M3 PRO group and K group (P=0.004, P=0.033), but no significant difference between MTwo group and M3 PRO group (P=0.451). Changes of root canal working length: There were significant differences among the three groups (P=0.037, P=0.037, P=0.000). M3 PRO group showed a minimal change while K group exhibited significant changes. Conclusion:MTwo nickel-titanium engine driven system with #10/04 file preform a high efficiency in glide path preparation, whilst M3 PRO nickel-titanium engine driven system show less changes in working length. Under the circumstances where the total time required for root canal preparation is not significant, M3 PRO nickel-titanium engine driven system is more suitable for root canal preparation in moderate curved root canals.

Objective @#To compare the shaping ability of 3 different nickel (Ni)-titanium (Ti) systems in simulated root canals in resin and to provide a reference for clinicians.@*Methods@#Forty-eight resin blocks were prepared using the F360 (Komet, Brasseler GmbH & Co., Lemgo, Germany) (Group 1), F6 SkyTaper (20/06) (Komet, Brasseler GmbH & Co., Lemgo, Germany) (Group 2), F6 SkyTaper (25/06) (Komet, Brasseler GmbH & Co., Lemgo, Germany) (Group 3) and Reciproc R25 systems (VDW, Munich, Germany) (Group 4) (n=12 canals/group). The images taken before and after preparation were superimposed and analyzed by Adobe Photoshop v7.0. The amount of resin removed by each system was measured, and the centering ability was assessed. The data were statistically analyzed using SPSS 20.0.@*Results @#At the 1 mm point, the transportation in Group 4 [(0.10 ± 0.03) mm] was significantly greater than that in Groups 2 [(0.05 ± 0.03) mm] and 3 [(0.05 ± 0.03) mm] (P < 0.05). At the 8 mm and 9 mm points, the transportation values in Group 4 [(0.12 ± 0.06) mm and (0.13 ± 0.05) mm] were significantly higher than those in Groups 2 [(0.05 ± 0.05) mm and (0.05 ± 0.05) mm] and 3 [(0.05 ± 0.04) mm and (0.06 ± 0.05) mm] (P < 0.05). At the 10 mm point, the transportation was significantly greater in Group 4 [(0.13 ± 0.06) mm] than in Group 2 [(0.06 ± 0.06) mm].@*Conclusion@#F6 SkyTaper exhibits better centering ability than Reciproc.

ABSTRACT

Objetivo: Comparar, en conductos curvos simulados de Endo Training Blocks, la conformación alcanzada por las limas ProGlider y WaveOne Gold Glider. Materiales y métodos: Se utilizaron 20 Endo Training Blocks con conductos curvos. Los conductos fueron teñidos con tinta china. Los bloques fueron divididos al azar en dos grupos, A y B, de 10 bloques cada uno. En el grupo A los conductos se permeabilizaron y conformaron con limas ProGlider, y en el grupo B, con limas WaveOne Gold Glider, accionadas con movimiento rotatorio y reciprocante, respectivamente. Para realizar la evaluación, los conductos se dividieron en tres tercios (cervical, medio y apical), para lo cual se colocó debajo de cada bloque una hoja milimetrada, y en esa posición fueron fotografiados. Se observaron las imágenes y se estableció la siguiente categorización: 0, no conformado; 1, conformación parcial; 2, conformado. La comparación entre los dos grupos dentro de cada tercio fue realizada por medio de la prueba de Chi cuadrado o de la prueba exacta de Fisher, según correspondiera. La comparación entre tercios dentro de cada grupo fue efectuada con la prueba de Friedman. En ambos casos se fijó el nivel de significancia en P<0,05. Resultados: En los tres tercios evaluados, el WaveOne Glod Glider mostró mejores resultados que el ProGlider, obervándose diferencias estadísticas significativas entre los grupos dentro de cada tercio. La comparación entre tercios dentro de cada grupo mostró, en el grupo A, ausencia de diferencias significativas entre los tercios cervical y medio, en tanto el tercio apical fue significativamente diferente a los otros dos; en el grupo B hubo diferencias estadísticas significativas entre el tercio cervical y el medio, mientras no las hubo entre el apical y los otros dos. Conclusiones: En los tres tercios evaluados, el WaveOne Gold Glider mostró mejores resultados que el ProGlider (AU)


Aim: To compare in simulated curved canals of Endo Training Blocks, the shaping ability of ProGlider and WaveOne Gold Glider files. Materials and methods: Twenty Endo Training Blocks with curved canals were used. The simulated canals were dyed with China ink. The blocks were randomly assigned to two groups, A and B, of 10 blocks each. Group A canals were shaped with ProGlider files and group B ones with WaveOne Gold Glider files, operated with rotary and reciprocating motion respectively. For the evaluation, the canals were divided into three thirds (cervical, middle and apical). A sheet of graft paper was placed under each block and they were photographed. The images were observed and the following score was established: 0, not shaped; 1, partially shaped; 2, shaped. The comparison between the two groups within each third was made by means of the chi-square test or Fisher's exact test as appropriate. The comparison between thirds within each group was made by means of Friedman's test. In both cases, the level of significance was set at P<0.05. Results: WaveOne Gold Glider showed better results than ProGlider in the three evaluated thirds. Statistical significant differences were detected between groups in each third. The comparison between thirds within each group showed, in group A, no significant differences between the cervical and middle thirds, while the apical third presented significant differences with the other two. In group B, significant differences were observed between the cervical and middle thirds, while there were none between the apical and the other two thirds. Conclusions: In the three thirds evaluated, WaveOne Gold Glider showed better results than ProGlider (AU)

ABSTRACT

Objective To discuss the application value of new non-invasive dynamic monopole radiofrequency (RF) system in body shaping.Methods 26 patients were included in this study,all female,aged 31 to 56 years.Application of new type of dynamic monopole radio frequency therapeutic apparatus (UK BTL Exilis therapeutic apparatus) was used to treat abdominal fat,proud flesh between the waist,back,hips,thighs and legs,upper arm area shape and the skin four times,with each time interval of 7~10 days.The patients filled in the subjective preliminary assessment questionnaire of satisfaction.Results All the patients had varying degrees of improvement in relaxation and partial fat reduction,with 76.9% of satisfactory rate.No adverse reactions and no skin burns occured.Conclusions The new non-invasive dynamic monopole RF system can be used to improve the degree of relaxation and local fat reduction in the body molding,with high satisfaction.

ABSTRACT

60 curved root canals of permanent teeth with angles of curvature ranging from 15°to 40°(Schneider's methodology)were prepared using the instruments of Hyflex CM(HC) and ProTaper(PT) Universal respectively(n=30).Using standardized pre-and post-instrumentation paralleling periapical radiographs,canal curvature was determined by image analysis software and the clinical shaping effect of Hyflex CM and ProTaper rotary NiTi files were compared.The canal curvature in group HC and PT decreased by 4.54°±3.25° and 5.63°±3.84° respectively(between pre-and post-treatment in both groups,P0.05).Hyflex CM can meet the clinical necessity for the instrumentation of curved root canals.

ABSTRACT

Objective To investigate the effects of angle and length of lightwand in three-dimensional reconstruction images of the upper airway based on computed tomography.Methods A total of 245 patients,aged 18-86 years,were scanned by helical computed tomography in the head and neck.Reconstructed images,obtained by Aw4.4 Workstation,were printed out by the ratio of 1 ∶ 1.The tracheal tube was used to investigate the anatomic structure of the upper airway.The length L and angle α of the tracheal tube were recorded.And two distances (D1,D2) and one angle (β) were recorded:D1,the distance between the upper central incisor and root of epiglottis;D2,the distance between the root of epiglottis and midpoint of glottis;angle β,the angle of line D1 and D2.Results The oα value in male patients was significant higher than that in female patients:(70.02 ± 15.49)° vs.(55.84 ± 14.70)°.The β value in male patients was significant higher than that in female patients:(129.74 ± 14.82)° vs.(120.40 ± 14.19)°.The L value in male patients was significant higher than that in female patients:(12.80 ± 0.85) cm vs.(11.50 ± 0.65) cm.The D1 and D2value in male patients were significant higher than those in female patients:(7.53 ± 0.66) cm vs.(6.76 ± 0.53) cm,(3.13 ± 0.52) cm vs.(2.45 ± 0.32) cm.There were significant differences (P < 0.05).The 245 patients were classified to 2 groups by using the function of hierarchical cluster analysis in the statistical software.The α value in group 2 was significant higher than that in group 1:(75.45 ± 9.53)° vs.(48.17 ± 9.29)°.The 3 value in group 2 was significant higher than that in group 1:(136.27 ± 9.86)° vs.(112.31 ± 8.64)°.The age in group 2 was significant higher than that in group 1:(53.92 ± 19.64) years vs.(36.93 ± 17.21) years.Correlation analysis showed that in 245 patients,angle α was positively correlated to angle β and age (P < 0.05),and angle β was positively correlated to age (P < 0.05).Conclusions The bent length and angle on lightwand can be affected by both the patients' gender and ages.

ABSTRACT

Objective To discuss the feasibility of using 3D printing-assisted precise microcatheter shaping technology in the embolization of intracranial aneurysms.Methods From November 2015 to April 2016,a total of 13 patients with intracranial aneurysm were treated with interventional embolization therapy.During the performance of the procedure,3D printing-assisted precise microcatheter shaping technology was employed.The accuracy of microcatheter position and the stability of the microcatheter were used to evaluate the shaping effect.Results During the performance of interventional embolization,the microcatheter was precisely placed to the right site in all 13 patients with intracranial aneurysm,the catheter tip position remained stable and the aneurysm was densely obstructed.In ten patients,no complications such as rupture of aneurysm occurred,and postoperative recovery was fine.One patient,whose Hunt-Hess classification was grade V,died after the treatment.One patient developed rupture of aneurysm during the procedure,which was considered to occur probably during the anesthesia induction process.One patient developed bleeding due to re-rupture of the aneurysm before the procedure.Conclusion With the help of 3D printing-assisted precise microcatheter shaping technology,the precisely shaped microcatheter can be smoothly inserted to,and can keep stable in,the right site,which is helpful for successfully accomplishing the operation and effectively reducing the occurrence of procedure-related complications.

ABSTRACT

Modified constraint-induced movement therapy (mCIMT), which is modified from constraint-induced movement therapy (CIMT), is used in hemiplegics for rehabilitation. It may be used in the selected patients in certain function and age. mCIMT is more flexible than CIMT, which is mainly consisted of intervention movement, intensity, and the time of training and limitation. mCIMT is also used with other rehabilitation methods. There are various assessment for the evaluation of upper extremity motor function. By using Contract and Mo-tor Activity Log, researchers can better guarantee the curative effect.

Objective:To investigate the shaping and cleaning ability of Reciproc(RE),OneShape(OS)and ProTaper(PT)in oval-shaped root canal preparation.Methods:The canals of 57 distal roots of mandibular first molars were randomly divided into 3 groups and prepared by RE,OS and PT respectively.The preparation time was recorded.After instrumentation,all root canal walls were examined under SEMand evaluated by scores of debris and smear layer.The 3D data and images of the roots of pre-and post-preparation were con-structed by Micro-CT scanning.Results:Group RE and OS showed lower scores of debris and smear layer than those in group PT at the apical level(P <0.05),and less preparation time (P <0.05).Before and after preparation,the changes of root canal morphological pa-rameters (including volume,surface,area,major diameter and minor diameter)in OS group were less than those in group RE and PT(P<0.05).The changes of canal curvature in group PT was larger than that in group RE and OS (P <0.05).Conclusion:RE and OS are more effective than PT in debridement and faster in root canal preparation.RE and OS can maintain the centering of original canal better than PT in the apical part of the root canal.The shaping ability of OS is weaker than that of RE and PT.

ABSTRACT

Objective To study the clinic value of digital three-dimensional shaping titanium mesh in repairing skull defects and reconstructing temporal muscle attachment points. Methods Eighty-two patients with skull defects from December 2013 to January 2016 were enrolled in this study. They were divided into two groups according to patients' will with 41 patients in each group. The patients in control group received traditional hand-shaping titanium mesh repair, the patients in observation group received individual digital forming technology to produce titanium mesh repair and reconstruct the temporal muscle attachment points at the titanium mesh temporal corresponding position. The rate of appearance satisfaction, operation time, the number of titanium nail and the rate of postsurgery adverse reaction was compared between the two groups. Results The rate of appearance satisfaction in observation group was significantly higher than that in control group: 97.56%(40/41) vs. 80.49%(33/41), P<0.05. The operation time and the number of titanium nail and in observation group was significantly lower than that in control group:(8.9 ± 1.6) h vs. (12.4 ± 2.4) h, (10.3 ± 0.9) pieces vs. (11.8 ± 1.8) pieces, P < 0.01. The rate of postsurgery adverse reaction in observation group was significantly lower than that in control group: 0 vs. 9.8%(4/41), χ2 = 4.21, P < 0.05. Conclusions The operation of repairing skull defects and reconstructing temporal muscle attachment points with digital three-dimensional shaping titanium mesh is simple. It can shorten the operation time, decrease the postsurgery adverse reaction, and increase the appearance satisfaction.

ABSTRACT

The aim of this study was to compare the percentage of non-instrumented area of root canals prepared with different enlargements using single-file reciprocating systems (Reciproc and WaveOne) and a conventional multi-file rotary (BioRaCe) system by micro-computed tomographic analysis. Thirty mesial roots of mandibular molars with moderate curvature (10° to 20°) presenting a type II Vertucci canal configuration and similar internal volume were chosen and scanned at an isotropic resolution of 14.16 µm. The sample was assigned to 3 groups (n=10) according to the system used for root canal preparation: Reciproc, WaveOne, and BioRaCe groups. Second and third scans were taken after the canals were prepared with instruments sizes 25 and 40, respectively. The recorded images of the surface area voxels of the canals, before and after preparation were examined from the furcation level to the apex to quantify the non-instrumented surface. Statistical data were compared using GLM for repeated-measures with a significance level set at 5%. Instrumentation systems did not influence the percentage of untouched root canal surfaces (p=0.690) whilst a significant reduction in the percentage of static voxels was observed after the enlargement of the root canal (p=0.010) in all groups (p=0.507). None of the systems was able to prepare the entire surface area of the mesial root canal of mandibular molars. The increased final apical size resulted in a significant positive effect on the shaping ability of the tested systems.


Resumo O objetivo deste estudo foi comparar a porcentagem de área não-instrumentada de canais radiculares preparados com diferentes ampliações utilizando sistemas reciprocantes de lima única (Reciproc e WaveOne) e um sistema rotatório convencional de múltiplas limas (BioRaCe) usando a análise da micro tomografia computadorizada. Trinta raízes mesiais de molares inferiores com curvatura moderada (10º a 20º) apresentando a configuração classe II de Vertucci e semelhança de volume do canal foram selecionadas e escaneadas em uma resolução isotrópica de 14,16 µm. A amostra foi dividida em 3 grupos (n=10) de acordo com o sistema utilizado para a preparação do canal radicular: grupo Reciproc, grupo WaveOne e grupo BioRaCe. Segundo e terceiro escaneamentos foram realizados após os canais serem preparados com instrumentos tamanhos 25 e 40, respectivamente. As imagens registradas de voxels da área de superfície dos canais, antes e após o preparo, foram examinadas desde o nível da furca até o ápice para quantificar a superfície não instrumentada. Os dados foram comparados estatisticamente através de GLM para medidas repetidas com um nível de significância de 5%. Os sistemas de instrumentação não influenciaram o percentual de superfícies não tocadas dos canais radiculares (p=0,690), enquanto que uma redução significativa no percentual de voxels estáticos foi observada após o alargamento do canal radicular (p=0,010) em todos os grupos (p=0,507). Nenhum dos sistemas foi capaz de preparar toda a área de superfície do canal mesial dos molares inferiores. O aumento do tamanho final apical resultou num efeito positivo significativo sobre a capacidade de modelar dos sistemas testados.

ABSTRACT

The ideal preparation for a root canal is a continuously tapered funnel shape with the smallest diameter at the apex and the widest diameter at the canal orifice. The main parameters used to evaluate a technique or an instrument which has been developed for root canal preparation should be ‘shaping the root canal whilst protecting the curvature of the canal’. Various methods have been proposed for evaluation of shaping ability of instruments. A reliable standardization and proper methodology is necessary for proper positioning of samples for obtaining images before and after root canal preparation. All techniques have their own benefits and limitations.

Objective To analyze and compare the effect,safety and prognosis of early and late digital shaping titanium mesh cranioplasty after skull bone flap defect.Methods One hundred and twelve patients with craniocerebral trauma or cerebral hemorrhage treated by decompressive craniectomy from Jan.2012 to Dec.2013 were included.Seventy-six cases were treated by digital shaping titanium mesh cranioplasty after average 38.9 (32~ 52) d after decompressive craniectomy (early repair group),and thirty-six cases were given operation after 114.2 (90~153) d (advanced repair group).The operation time,skin flap free time,operation blood loss,complications,postoperative patients' activities of daily living (ADL) score,Fugl-Meyery score,neurological deficit score and the recovery of cognitive function of the two groups were compared and analyzed.Results Operation time of early repair group and late repair group were (92.33±13.71) min and (141.67±18.12) min,flap free time were (13.29± 3.43) min and (38.56±7.35) min,and operation bleeding volume were (352.36 ± 21.83) ml and (523.53±32.51) ml (P<0.01).The subdural effusion rate and the dural rupture rate during skin flap free of early repair group was significantly less than that of late repair group (P<0.01).ADL and Fugl-Meyery scores in early repair group were increased obviously compared with advanced repair group with significant difference (P<0.05 or P<0.01),while the neurological deficit score decreased significantly (P<0.05).The Montreal cognitive assessment suggested that cognitive function in early repair group was better than that of the late repair group with significant difference (P<0.05).Conclusions Early digital shaping titanium mesh cranioplasty after skull bone flap defect is better than late treatment with easy soft tissue separation,reduced bleeding and improved prognosis.Digital titanium mesh shaping can ensure precise fit with skull defect,significantly shorten the operation time,and reduce the operation risk.Early digital shaping titanium mesh cranioplasty may have promising clinical application.

Objective To evaluate the recent safety and long-term curative effect of double-pool shaping trabeculectomy with adjustable sutures in glaucoma. Methods A hundred and twenty patients with glaucoma were randomly divided into two groups, in which the control group in 62 patients underwent traditional trabeculectomy and the experimental group in 58 patients underwent double-pool shaping trabeculectomy with adjustable sutures. All patients were followed up for one year. Postoprative intraocular pressure, anterior chamber depth and filtering bleb were observed. Results In the experimental group, after the demolition of adjustable sutures, 3 eyes(5.17%) suffered from I degree shallow anterior chamber. By contrast, 8 eyes suffered from I degree shallow anterior chamber, and 1 eye (total 14.52%) suffered from Ⅱ degree shallow anterior chamber in the control group (P < 0.05). Following up in one year, the functional filtering bleb and intraocular pressure got better in the experimental group than in the control group (P < 0.05). Conclusion Double-pool shaping trabeculectomy with adjustable sutures is a safe and feasible method for glaucoma, in which it can effectively reduce intraocular pressure without complications.

Introducción: para prevenir la fractura de instrumentos endodonticos actualmente se siguen investigando materiales, técnicas y diseños los cuales buscan reducir su incidencia. La conformación de túnel liso (Glide Path) se ha propuesto con este fin, sin embargo su utilidad ha sido controvertida. Objetivo: comprobar teóricamente mediante la utilización de un análisis de elementos finitos, la utilidad de la conformación de túnel liso, antes de la utilización de limas rotatorias endodonticas de níquel-titanio. Métodos: se realizó una simulación numérica mediante un análisis de elementos finitos, para lo cual se construyeron modelos matemáticos de las limas rotatorias de níquel-titanio S1 y S2 de la serie Protaper®. Con el software Simulation multyphisics de Autodesk® se programaron las propiedades mecánicas de las limas y un límite de falla por fractura de 1270,588 MPa. Posteriormente se aplicó el torque recomendado por el fabricante y se fijo la punta del instrumento (sin conformación de túnel liso) aplicando allí restricciones en todos los grados de libertad.Para simular la realización de la conformación de túnel liso, se aplicaron restricciones en todos los grados de libertad a una determinada distancia de la punta del instrumento, simulando de esta forma que la punta del instrumento permanecía libre. Resultados: al fijar la punta de las limas, los esfuerzos máximos fueron 1545,77 MPa para la S1 y 1306,47 MPa para la S2, observando fractura de los instrumentos. Al fijar las limas a distancia de la punta no se observó fractura. Conclusiones: se demostró teóricamente que al impedir que se atrapen la punta de las limas, se previene su fractura y que dicho atrapamiento se evita con una conformación de túnel liso, por lo que se concluye que este procedimiento es útil para prevenir la fractura de los instrumentos rotatorios de níquel-titanio(AU)


Introduction: materials, techniques and designs continue to be studied with the purpose of reducing the incidence of fracture in endodontic instruments. Shaping of the glide path has been proposed for this end; however, its usefulness has been a matter of controversy. Objective: based on the finite element method, carry out a theoretical analysis of the usefulness of shaping the glide path before using rotary nickel-titanium endodontic files. Methods: numerical simulation was performed based on finite element analysis, to achieve which mathematical models were built of rotary nickel-titanium files S1 and S2 of the Protaper™ series. Autodesk™ Simulation Multiphysics software was used to program the mechanical properties of the files, as well as a fracture failure limit of 1270.588 MPa. The torque recommended by the manufacturer was then applied and the instrument tip fixed into place (without shaping the glide path) with restrictions on all degrees of freedom. In order to simulate glide path shaping, restrictions were applied to all degrees of freedom at a certain distance from the instrument tip, thus simulating that the instrument tip remained free. Results: on fixing the file tips, maximum efforts were 1545.77 Mpa for S1 and 1306.47 Mpa for S2. Instrument fracture was observed. When files were fixed at a distance from the tip, no fracture was observed. Conclusions: it was theoretically demonstrated that when file tips are prevented from being caught, fracture is prevented as well, and such catch is avoided by shaping the glide path. It is therefore concluded that the procedure is useful to prevent the fracture of rotary nickel-titanium instruments(AU)
